Self Declaration Form

​What is a Self Declaration Form?

The ‘self declaration form’ is a form in which we ask you to declare any criminal warnings, cautions and convictions that you may have and to agree to keep us informed of any subsequent warnings, cautions or convictions.  It also provides the institution with consent for applicants who have signed up for the DBS Update Service.

If a conviction is indicated, please provide in a sealed envelope a letter detailing/explaining the content and circumstances of the conviction/caution in order to assist with the processing of your application.

Do I need to complete a Self Declaration form?

You should complete and return the form if;

  • You have been offered a place and a DBS check forms part of your offer conditions
  • You have had a break in study and are returning to continue your programme
  • You already have a DBS check and want to give us consent to check your Update Service
